Understanding Lymphedema & Why Early Treatment Matters

Lymphedema is a chronic condition caused by damage to or malfunction of the lymphatic system — the network of vessels and nodes responsible for draining excess fluid from your body's tissues. When lymph drainage is impaired, protein-rich fluid accumulates in the soft tissue, causing progressive swelling, skin changes, heaviness, and increased infection risk.

Lymphedema is classified as primary (congenital or genetic lymphatic abnormality) or secondary (acquired damage from cancer treatment, surgery, infection, trauma, or obesity). Secondary lymphedema is by far the most common form, accounting for approximately 99% of cases worldwide.

Lymphedema is described as a "hidden epidemic." The WHO estimates 1 in 30 people worldwide live with it — affecting more Canadians than HIV, Parkinson's disease, MS, and muscular dystrophy combined. There is currently no cure, but it is highly manageable with proper physiotherapy.

Early intervention is critical. Without treatment, lymphedema progresses through stages with increasingly difficult-to-reverse tissue changes. Recognising Lymphedema — Signs, Symptoms & Staging

Lymphedema can affect any part of the body. Recognising the early signs and seeking physiotherapy promptly leads to significantly better outcomes.

Early Warning Signs

  • A feeling of heaviness, fullness or tightness in a limb
  • Persistent puffiness or swelling that doesn't resolve overnight

Complete Decongestive Therapy — The Gold Standard

CDT is the internationally recommended first-line treatment for lymphedema, endorsed by the International Society of Lymphology and Cancer Care Ontario. At RCP Health, we deliver both phases of CDT as a fully integrated program.

Phase 1

Intensive Decongestive Phase

Daily or near-daily sessions focused on actively reducing limb volume. Typically 2–4 weeks depending on severity and stage.

The Four Pillars of CDT

Manual Lymphatic Drainage (MLD)

A specialised, gentle massage technique using rhythmic, light pressure to stimulate lymphatic vessels and redirect fluid away from blocked areas through alternative drainage pathways. Performed only by trained physiotherapists.

Compression Bandaging

Multi-layer, short-stretch bandaging worn between MLD sessions creates a low-pressure environment that prevents fluid from re-accumulating. Progresses to fitted compression garments in Phase 2.

Decongestive Exercise

Specific therapeutic exercises performed while wearing compression activate the muscle pump, enhancing lymphatic flow. Programs are tailored to your fitness level, diagnosis, and current treatment stage.

How We Assess & Monitor Lymphedema at RCP Health

Accurate assessment is essential for staging lymphedema, tracking treatment progress, and determining when compression garments are ready for fitting.

Circumferential Limb Measurement

Sequential measurements at standardised points along the limb to calculate volume and compare symmetry with the unaffected limb. The primary tool for tracking progress through CDT.

Bioimpedance Spectroscopy (BIS)

Advanced technology that measures the electrical resistance of tissue to detect changes in fluid levels — capable of identifying lymphedema at Stage 0 before visible swelling appears.

Skin & Tissue Assessment

Palpation for tissue fibrosis, pitting response, skin texture changes, and Stemmer's sign — the inability to pinch a fold of skin at the base of the second toe or finger, a positive indicator of lymphedema.

Functional & Quality of Life Assessment

Validated tools including the Lymphoedema Quality of Life (LYMQOL) questionnaire and functional movement assessment to capture the full impact of lymphedema on daily activities.

It is never too late to begin lymphedema treatment. While early intervention produces the best outcomes, CDT is effective even for long-standing lymphedema. Patients with chronic, fibrotic Stage II and III lymphedema can still achieve meaningful volume reduction, improved skin condition, and significantly better quality of life. The treatment approach and goals will be adjusted to your current presentation.
Regular oedema from injuries or prolonged standing typically resolves with elevation and rest within 24–48 hours. Lymphedema is persistent — it does not resolve overnight, does not respond to elevation alone, and often increases over time without treatment. Key indicators of lymphedema include a history of lymph node surgery or radiation, swelling that is worse at the end of the day, and a positive Stemmer's sign. A physiotherapy assessment will determine the cause of your swelling.
