The Ontario Bundled Care Program is a provincial Ministry of Health initiative where hospitals partner with community physiotherapy clinics to provide coordinated, fully-funded post-surgical rehabilitation for patients undergoing joint replacement surgery.

You undergo your primary hip, knee, or shoulder replacement at a participating Ontario hospital.

Your hospital case coordinator provides your Bundled Care package and a referral to a partnered clinic.

Conditions Commonly Covered Under Bundled Care
The Ontario Bundled Care Program covers three primary joint replacement procedures. All must be first-time (primary) surgeries at a participating Ontario hospital.
Total Knee Replacement
For patients with severe knee osteoarthritis or joint damage. Rehabilitation targets quadriceps re-activation, range of motion, swelling management, and stair function.

Our Role in Ontario Bundled Care
RCP Health acts as your community rehabilitation partner — receiving hospital referrals, coordinating with your surgical team, and delivering your full physiotherapy program under the Bundled Care pathway.

Hospital Coordination
We liaise directly with your referring hospital, care coordinator, and surgeon. We receive your Bundled Care referral, confirm eligibility, and communicate your progress back to the hospital throughout your program.

Registered Physiotherapists Only
Every Bundled Care assessment and treatment at RCP Health is delivered by a College of Physiotherapists of Ontario (CPO) registered clinician with post-surgical rehabilitation experience.

Direct Billing to Hospital
We submit all billing directly to your referring hospital under the Bundled Care agreement. There is no paperwork, no insurance claims, and no cost for eligible patients.

Seamless Discharge-to-Recovery Pathway
We are committed to receiving patients within the recommended 2-week post-discharge window and providing the intensive early rehabilitation that evidence shows is critical to optimal outcomes.

- How many physiotherapy sessions are covered under Bundled Care?
- The program covers an initial assessment plus up to 8 treatment sessions focused on improving mobility, strength, and function through evidence-based exercise therapy.
- Can I receive Bundled Care physiotherapy at home?
- Yes. RCP Health offers home-based physiotherapy for eligible Bundled Care patients who have difficulty travelling to the clinic. Our registered physiotherapists serve Oakville, Burlington, and Halton Region.
- Does RCP Health accept Bundled Care referrals from all Ontario hospitals?
- RCP Health partners with participating hospitals in the Halton and GTA West region. If you have received a Bundled Care referral, contact us to confirm eligibility and book your first appointment.
- Who does NOT qualify for the Ontario Bundled Care Program?
- Patients who had revision surgery, bilateral surgery, surgeries at non-participating hospitals, or surgeries outside Ontario do not qualify. Patients with major surgical complications may also be ineligible.
- What is the difference between Bundled Care and the Community Physiotherapy Clinic Program?
- Bundled Care is funded by the hospital and is specific to post-joint-replacement rehabilitation. The CPC Program is a separate OHIP-funded program for patients discharged from hospital or on Ontario Works or ODSP. Both programs are available at RCP Health.
