Lymphedema
Management in Oakville
Evidence-based Complete Decongestive Therapy — the gold standard for reducing swelling, restoring function, and empowering long-term self-management of lymphedema.
Understanding Lymphedema & Why Early Treatment Matters
Lymphedema is a chronic condition caused by damage to or malfunction of the lymphatic system — the network of vessels and nodes responsible for draining excess fluid from your body's tissues. When lymph drainage is impaired, protein-rich fluid accumulates in the soft tissue, causing progressive swelling, skin changes, heaviness, and increased infection risk.
Lymphedema is classified as primary (congenital or genetic lymphatic abnormality) or secondary (acquired damage from cancer treatment, surgery, infection, trauma, or obesity). Secondary lymphedema is by far the most common form, accounting for approximately 99% of cases worldwide.
Lymphedema is described as a "hidden epidemic." The WHO estimates 1 in 30 people worldwide live with it — affecting more Canadians than HIV, Parkinson's disease, MS, and muscular dystrophy combined. There is currently no cure, but it is highly manageable with proper physiotherapy.
Early intervention is critical. Without treatment, lymphedema progresses through stages with increasingly difficult-to-reverse tissue changes. At RCP Health, our trained physiotherapists deliver evidence-based Complete Decongestive Therapy (CDT) — the internationally recognised gold standard — to reduce limb volume, restore function, and empower long-term self-management.
Recognising Lymphedema — Signs, Symptoms & Staging
Lymphedema can affect any part of the body. Recognising the early signs and seeking physiotherapy promptly leads to significantly better outcomes.
⚠️ Early Warning Signs
- A feeling of heaviness, fullness or tightness in a limb
- Persistent puffiness or swelling that doesn't resolve overnight
- Clothing, jewellery or shoes feeling tighter on one side
- Aching, discomfort or restricted range of motion
- Skin that pits when pressed (pitting oedema)
- Recurring skin infections (cellulitis) in the affected limb
- Visible difference in limb size between left and right
🔬 Advanced Symptoms
- Non-pitting oedema — swelling that no longer responds to elevation
- Skin thickening, hardening or fibrosis (brawny oedema)
- Leathery or cobblestone skin texture (hyperkeratosis)
- Fluid leaking through the skin (lymphorrhoea)
- Deep skin folds and skin fold complications
- Significant limb enlargement affecting mobility and daily life
- Reduced quality of life, anxiety and depression related to the condition
The International Society of Lymphology (ISL) Staging System
Complete Decongestive Therapy — The Gold Standard
CDT is the internationally recommended first-line treatment for lymphedema, endorsed by the International Society of Lymphology and Cancer Care Ontario. At RCP Health, we deliver both phases of CDT as a fully integrated program.
Intensive Decongestive Phase
Daily or near-daily sessions focused on actively reducing limb volume. Typically 2–4 weeks depending on severity and stage.
- Manual Lymphatic Drainage sessions
- Multi-layer compression bandaging
- Decongestive exercises during treatment
- Skin care assessment and instruction
- Volume measurement and progress tracking
Maintenance & Self-Management Phase
Preserving the gains achieved in Phase 1 through compression garment use, home exercise, and periodic follow-up sessions.
- Compression garment measurement and prescription
- Home MLD self-massage technique training
- Ongoing exercise program
- Skin care maintenance protocol
- Periodic reassessment and plan adjustments
The Four Pillars of CDT
Manual Lymphatic Drainage (MLD)
A specialised, gentle massage technique using rhythmic, light pressure to stimulate lymphatic vessels and redirect fluid away from blocked areas through alternative drainage pathways. Performed only by trained physiotherapists.
Compression Bandaging
Multi-layer, short-stretch bandaging worn between MLD sessions creates a low-pressure environment that prevents fluid from re-accumulating. Progresses to fitted compression garments in Phase 2.
Decongestive Exercise
Specific therapeutic exercises performed while wearing compression activate the muscle pump, enhancing lymphatic flow. Programs are tailored to your fitness level, diagnosis, and current treatment stage.
Skin Care & Education
Meticulous skin care prevents cellulitis — a serious infection that significantly worsens lymphedema. We provide comprehensive education on skin care protocols, self-monitoring, and recognising early warning signs of infection.
Types of Lymphedema & Related Conditions We Treat
RCP Health provides specialist lymphedema management for a wide range of primary and secondary causes across all stages.
Breast Cancer-Related Lymphedema
The most common form of secondary lymphedema, affecting up to 30% of breast cancer survivors following axillary lymph node dissection or radiation.
Cancer Rehabilitation at RCP HealthLower Limb Lymphedema
Leg swelling following pelvic cancer treatment (cervical, prostate, colorectal), vascular surgery, or trauma. Affects mobility and daily function significantly.
Upper Limb Lymphedema
Arm swelling most commonly following breast cancer treatment, head and neck cancer surgery, or melanoma treatment. CDT produces excellent results at all stages.
Primary (Congenital) Lymphedema
Present from birth or developing in adolescence or early adulthood due to abnormal lymphatic vessel development. Milroy disease and Meige disease are common forms.
Post-Surgical Lymphedema
Swelling following orthopaedic surgery, vascular procedures, or any surgery involving lymph node removal. Early physiotherapy significantly improves outcomes.
MSK Physiotherapy at RCP HealthHead & Neck Lymphedema
100% of patients treated for head and neck cancer develop some degree of lymphedema. Specialised internal and external MLD techniques are used for this complex presentation.
Cancer Rehabilitation at RCP HealthObesity-Related Lymphedema
Excess weight places significant strain on the lymphatic system. CDT combined with weight management strategies is highly effective for this growing patient population.
Pregnancy-Related Oedema
Persistent postpartum swelling that may indicate lymphatic involvement. Physiotherapy assessment differentiates lymphedema from venous oedema and guides appropriate management.
Prenatal & Postnatal PhysiotherapyAt-Home Lymphedema Management
For patients with mobility limitations, our physiotherapists provide at-home CDT visits across Oakville and surrounding areas.
At-Home Physiotherapy ServicesHow We Assess & Monitor Lymphedema at RCP Health
Accurate assessment is essential for staging lymphedema, tracking treatment progress, and determining when compression garments are ready for fitting.
Circumferential Limb Measurement
Sequential measurements at standardised points along the limb to calculate volume and compare symmetry with the unaffected limb. The primary tool for tracking progress through CDT.
Bioimpedance Spectroscopy (BIS)
Advanced technology that measures the electrical resistance of tissue to detect changes in fluid levels — capable of identifying lymphedema at Stage 0 before visible swelling appears.
Skin & Tissue Assessment
Palpation for tissue fibrosis, pitting response, skin texture changes, and Stemmer's sign — the inability to pinch a fold of skin at the base of the second toe or finger, a positive indicator of lymphedema.
Functional & Quality of Life Assessment
Validated tools including the Lymphoedema Quality of Life (LYMQOL) questionnaire and functional movement assessment to capture the full impact of lymphedema on daily activities.
Pelvic Floor Screening
For patients with lower limb or genital lymphedema following pelvic cancer treatment, we screen for associated pelvic floor dysfunction which frequently co-occurs and benefits from integrated treatment.
Compression Garment Measurement
Precise limb measurements for custom compression garment prescription. We advise on garment class, fabric, and style appropriate to your diagnosis, lifestyle, and occupation.
