What I See in Practice β€” and What Surprises Many Patients

In clinical practice, I frequently see patients who have been managing a stiff, painful joint for months β€” sometimes years β€” who have been dutifully doing their stretches without meaningful improvement. When I assess them manually, what I find is not simply tightness in the muscles they have been stretching. Often, there is a specific joint segment that has lost its normal arthrokinematic movement β€” the small, internal gliding and rolling motions that need to occur for a joint to move through its full range without compensation. No amount of stretching addresses that directly. A targeted joint mobilisation can restore that accessory movement within a single session, and patients are often genuinely surprised by how different the joint feels immediately afterward.

A nuanced clinical insight that I think is under-discussed: the nervous system is often the primary driver of perceived stiffness, not the tissue itself. After injury or chronic overload, the nervous system upregulates protective tone in surrounding muscles, creating the sensation of tightness even when the tissue is structurally capable of lengthening. Skilled manual therapy β€” particularly sustained soft tissue techniques applied with appropriate pressure β€” can interrupt that neural protective response. This is one reason why patients sometimes feel a release of tension in areas the therapist did not directly touch.

When Manual Therapy Fits β€” and When to Look Further

  • Acute trauma with unresolved swelling or suspected structural damage requiring imaging first
  • A clinical presentation that is not responding as expected after a reasonable trial of appropriate treatment
